Is cosmetic surgery painful?

The word "surgery" conjures up images of incisions and, consequently, pain. In fact, in cosmetic surgery, even procedures without incisions, such as double eyelid surgery (using sutures), dimple creation (using sutures), and correction of protruding ears (using sutures), require anesthesia to ensure optimal results. If the patient is groaning in pain during surgery, it will affect the doctor's concentration, and rushing the procedure can lead to adverse consequences. Therefore, the method and effectiveness of anesthesia are key considerations for doctors when planning surgery.

Of course, it's impossible for surgery to be completely painless. The injection of local anesthetic causes only a slight pain, similar to an intramuscular injection in the buttocks. The pain disappears once the anesthetic is administered. Sometimes, during surgery, touch and pain sensations are separated; one may feel touch but not pain. After surgery, the pain is limited because the anesthetic wears off gradually and the bandages also help. Sensitive patients can usually manage with painkillers.

Does anesthetic affect the brain during cosmetic surgery?

Cosmetic surgery primarily affects young and middle-aged women, who have much higher requirements for anesthetic safety than for regular surgery. Since the surgical sites are mostly on the skin's surface, where sensation is highly sensitive, local anesthesia is more common, generally requiring neither muscle relaxation nor deep general anesthesia.

The principles for choosing local anesthetics in cosmetic surgery are: safety, effectiveness, and low toxicity. Anesthetics that may cause allergic reactions are generally avoided. Drugs with rapid onset, strong penetration, wide diffusion, and low toxicity are typically used. These drugs are generally metabolized in the liver and, within safe dosage ranges, will not affect the central nervous system, i.e., the brain's memory, thinking, or reaction abilities. Even when using intravenous anesthetics, their effects are rapid and short-lived, selectively inhibiting brain connections and thalamic projections to the neocortex. Therefore, consciousness remains partially, while pain sensation is significantly and completely eliminated. The anesthetic effect wears off completely, leaving no sequelae or discomfort.

For larger surgeries, an anesthesiologist must be consulted for guidance or to perform the procedure.

Will cosmetic surgery leave unsightly scars?

In general surgery, the primary focus is on relieving the patient's pain and removing the cause of the problem. The incision must be easily accessible, with maximum surgical exposure to meet the surgical requirements and facilitate postoperative dressing changes. Therefore, the incision location and scar management are not major concerns, often leading to disagreements with the patient. However, in cosmetic surgery, the situation is the opposite. Both doctors and patients are highly concerned about the location and size of the incision and the appearance of postoperative scars. When the procedure allows, incisions are made at the hairline, in skin folds, or hidden under clothing. Because specialized surgical instruments used in cosmetic surgery are employed, the required incision length is very short, and doctors perform scar-free procedures to minimize surgical scarring. Surgical incisions are necessary to ensure surgical results and prevent scarring. The edges of the incision are sharp and clean, and with careful suturing, scarring can be minimized. Some surgical incisions are only 3-4 mm in size and designed in inconspicuous locations; these can be called scarless incisions. Because scarless incisions require a high level of skill, not all doctors can perform them. Some scarless methods can leave irregular scars on the body, which are very difficult to repair. Among all surgeries, liposuction has the smallest incision, with the main surgical layer located in the subcutaneous fat layer. With careful handling by the doctor, it generally does not leave unsightly scars after the procedure.
